Schedule an Appointment

To schedule an appointment:

  • Choose the service.
  • Contact us to schedule an appointment. We will email or call you to schedule your appointment.
  • Our offices are conveniently located at:

    307 South Salem Street, Suite 302, Apex, NC 27502

  • After you schedule your appointment, you will receive an email acknowledgment, a copy of our policies and procedures, and nutrition consultation forms from Personalized Nutrition Inc.

  • If you are scheduling an appointment for a diagnosed medical condition, and/or plan to file for reimbursement from your insurance company, please have your physician fax your medical diagnosis and current pertinent lab work to (919) 367-0818.
    All information remains secure and confidential.

Policies and Procedures:

Appointments
Individual appointments are reserved just for you, and they are scheduled for a specific time. We usually give you a reminder phone call the day before to confirm your appointment. You will be charged $50.00 for late cancellations and missed appointments, unless we are notified at least 24 hours in advance by phone. You may reschedule, change or cancel your nutrition appointment day and/or time by calling (919) 367-0677 up to 24 hours before your scheduled appointment time. If you are late for your appointment, you forfeit the time missed. The appointment will end at the originally scheduled ending time. If you miss an appointment, you forfeit the time missed. However, the remaining time in the package will be honored.

Fees and Payment
Full payment for a nutrition package is expected at the first appointment. We accept cash, checks and most major credit cards. No refunds will be given. If we bill your insurance company for services rendered, and your visit is denied reimbursment, you will be billed by Personalized Nutrition, Inc., and payment is expected within 30 days of the date of bill/invoice. This may include, but is not limited to any applicable deductibles, co-pays, or any visits that are simply denied by your insurance company. You are responsible for paying for your nutrition appointments if your insurance denies them as a covered entity. Please call your insurance company prior to scheduling your appointment with us to determine if you have nutrition reimbursement benefits.

Consent for Services
We will ask you to sign a consent for services before your first appointment, agreeing that you understand and will adhere to our policies and procedures. Your signature also indicates that you accept nutrition counseling services and have been offered a copy of our privacy notice. We look forward to helping you achieve your health and nutrition goals.
